Change findInstancesInHierarchy to return implicit top instance (#1226) 

* Change FIRRTL-internal API, affecting only one corner case
* Make API more "DWIM" and consistent with other methods
* Add test cases for findInstancesInHierarchy
* Update Scaladoc
