std_fstream.h (xsputn): Don't call _M_destroy_pback...

* include/std/std_fstream.h (xsputn): Don't call _M_destroy_pback:
if output is at all possible (!_M_reading), cannot be active.
* include/std/std_fstream.h: Tweak comments to doxygen style.
