According to https://developer.apple.com/support/xcode/ the removed
annotations can't fire anymore, since the targets they would fire for
aren't supported anymore.

This reverts commit c861fe8a71e64f3d2108c58147e7375cd9314521.
Unfortunately, this use of hidden visibility attributes causes
user-defined specializations of standard-library types to also be marked
hidden by default, which is incorrect. See discussion thread on #131156.

_LIBCPP_HIDE_FROM_ABI everywhere (#131156)
This patch introduces `_LIBCPP_{BEGIN,END}_EXPLICIT_ABI_ANNOTATIONS`,
which allow us to have implicit annotations for most functions, and just
where it's not "hide_from_abi everything" we add explicit annotations.
This allows us to drop the `_LIBCPP_HIDE_FROM_ABI` macro from most
functions in libc++.

It doesn't serve much of a purpose since we can easily put its contents
inside __config. Removing it simplifies the modulemap once we are trying
to create a single top-level module.

In essence, this header has always been related to configuration of
the library but we didn't want to put it inside <__config> due to
complexity reasons. Now that we have sub-headers in <__config>, we
can move <__availability> to it and stop including it everywhere since
we already obtain the required macros via <__config>.

This patch runs clang-format on all of libcxx/include and libcxx/src, in
accordance with the RFC discussed at [1]. Follow-up patches will format
the benchmarks, the test suite and remaining parts of the code. I'm
splitting this one into its own patch so the diff is a bit easier to
review.

Our best guess is that the two syntaxes should have exactly equivalent
effects, so, let's be consistent with what we do in libcxx/include/.
I've left `#include "include/x.h"` and `#include "../y.h"` alone
because I'm less sure that they're interchangeable, and they aren't
inconsistent with libcxx/include/ because libcxx/include/ never
does that kind of thing.
Also, use the `_LIBCPP_PUSH_MACROS/POP_MACROS` dance for `<__undef_macros>`,
even though it's technically unnecessary in a standalone .cpp file,
just so we have consistently one way to do it.

We've stopped doing it in libc++ for a while now because these names
would end up rotting as we move things around and copy/paste stuff.
This cleans up all the existing files so as to stop the spreading
as people copy-paste headers around.

We always build the libraries in a Standard mode that supports noexcept,
so there's no need to use the _NOEXCEPT macro.

Currently, vendor-specific availability markup is enabled by default.
This means that even when building against trunk libc++, the headers
will by default prevent you from using some features that were not
released in the dylib on your target platform. This is a source of
frustration since people building libc++ from sources are usually not
trying to use some vendor's released dylib.
For that reason, I've been thinking for a long time that availability
annotations should be off by default, which is the primary change that
this commit enables.
In addition, it reworks the implementation to make it easier for new
vendors to add availability annotations for their platform, and it
refreshes the documentation to reflect the current state of the codebase.
Finally, a CMake configuration option is added to control whether
availability annotations should be turned on for the flavor of libc++
being created. The intent is for vendors like Apple to turn it on, and
for the upstream libc++ to leave it off (the default).

defeated my regular expressions.
We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

I am making this change now in anticipation, and will get it into 4.0, because (a) 4.0 is the first release with std::optional, and (b) I don't want to make an ABI-change later, when the user base should be significantly larger. Note that I didn't change std::experimental::bad_optional_access, because that's still specified to derive from std::logic_error.

Adapt implementation of Library Fundamentals TS optional into an implementation of N4606 optional.
- Update relational operators per http://wg21.link/P0307
- Update to requirements of http://wg21.link/P0032
- Extension: Implement trivial copy/move construction/assignment for `optional<T>` when `T` is trivially copyable.
Audit P/Rs for optional LWG issues:
- 2756 "C++ WP optional<T> should 'forward' T's implicit conversions" Implemented, which also resolves 2753 "Optional's constructors and assignments need constraints" (modulo my refusal to explicitly delete the move operations, which is a design error that I'm working on correcting in the 2756 P/R).
- 2736 "nullopt_t insufficiently constrained" Already conforming. I've added a test ensuring that `nullopt_t` is not copy-initializable from an empty braced-init-list, which I believe is the root intent of the issue, to avoid regression.
- 2740 "constexpr optional<T>::operator->" Already conforming.
- 2746 "Inconsistency between requirements for emplace between optional and variant" No P/R, but note that the author's '"suggested resolution" is already implemented.
- 2748 "swappable traits for optionals" Already conforming.
- 2753 "Optional's constructors and assignments need constraints" Implemented.
