* remote-utils.h (struct serial): Declare as opaque. Remove
include of "serial.h".
* Makefile.in (remote_utils_h): Update.
* monitor.h (struct serial): Declare as opaque. Remove include of
"serial.h".
(struct monitor_ops): Replace serial_t with `struct serial *'.
* monitor.c (monitor_desc): Ditto.
