Table Fixes

Update filenames in workflows.

Updated the filenames in the GH workflows.

machine.adoc: fix table title

- move title before the table
- replace redundant FS by VS

Resolves #1437

No need for the echos, which are somewhat nonstandard

No need for asciidoc in filenames therefore removing.

* Flatten changelog as v1.13

Integrated changelog for 1.13 with last document version changes which consisted of
integration of ratified specs.

Use mcycle, minstret, mhpmcountern to replace cycle, instret, hpmcountern in mcountinhibit spec. This will not changing any meaning because of "read-only shadow".

Resolves #1421

The c.zext.h prerequisites and operation were duplicated at the end of the c.sext.b section.
