quick-access: Update SM Diagram for Quick Access

Edit the SM Diagram to clarify that Quick Access is currently a
completely different conceptual path from the "AccessRegister" command.

Clarify that Error status is only clearable once BUSY is not set.

Remove the stray reference to 'prehalt' that was in the Access Reg command.
