simplify docs

- removes custom theme in favor of vendored "readable" theme
- moves remaining doc source files from gh-pages into master
- CDRIVER-3445 add NPS survey
- CDRIVER-3463 update google analytics
- CDRIVER-2833 fix the search on libmongoc/libbson sites, remove from
home page
