Change Module::ASTFile and ModuleFile::File => Optional<FileEntryRef>, NFC

Change `Module::ASTFile` and `ModuleFile::File` to use
`Optional<FileEntryRef>` instead of `const FileEntry *`. One of many
steps toward removing `FileEntry::getName`.
