faster loads, less chance of namespace pollution

QCD quantization sanity check should run after tile header is read, rather than after main header is read.

With this change, 3 files that that used to fail opj_dump now do not fail, since dump
only looks at main header. So, these files must be removed from black list.

fixes #128 . When setting rate for tiled compress, adjust for header size for each tile proportional

to how large the tile is.
