SPDX makes it a simpler diff.
I have audited the commit history of each file to ensure that they are
exclusively authored by IBM and thus we have the right to relicense.
The motivation behind this is twofold:
1) We want to enable experiments with coreboot, which is GPLv2 licensed
2) An upcoming firmware component wants to incorporate code from skiboot
and code from the Linux kernel, which is GPLv2 licensed.
I have gone through the IBM internal way of gaining approval for this.

At times we need to perform some cleanup activities when the Opal PCI
state machine that perform creset/freset/hreset (driven by
pci_slot_ops->run_sm which) of a slot completes. One example can be to
mark CAPP attached to a PHB, as deactivated when creset/freset of a
CAPI card slot is completed.
However the calls to pci_slot_ops->run_sm() is scattered through out
the code and patching each call site to check for the return value and
perform custom cleanup tacks is difficult.
Hence this patch introduces a new pci_slot_ops named
completed_sm_run() which should be called when pci_slot_ops->run_sm()
determines that the reset state machine is complete. This provides a
more centralized way to handle slot related cleanup activities.

The PCIe slot capability can be implemented in a root or switch
downstream port to set the maximum power a card is allowed to draw
from the system. This patch adds support for setting the power limit
when the platform has defined one.

Witherspoon introduced a new concept where one physical slot is shared
between two PHBs. Making a slot aware of its peer enables syncing
between them where necessary.

astbmc has some code to handle devices that are behind a "slot" on a
riser card that can't be added to the static slot tables for a system.
We probably want to use this code outside the slot table handling so
move it somewhere generic and rework it so slot table specifics aren't
buried inside it.

In P9 we get information about the physical PCIe slot topology
through the HDAT. As a rule we never directly consume the HDAT
inside of Skiboot and we always parse and incorporate the data
from HDAT into the Skiboot device tree.

This improves PHB reset and link training timing. Justifications and
reasons are included in the patch.
Polling frequencies are decreased from 100ms to 10ms.
Added is a new state called PHB4_SLOT_LINK_STABLE which is now needed
since the link training can be so fast that we touch config space too
quickly (PCIe spec requires 1 second between PERST de-assert and
device config space reads). We use this new state to sanity check the
PHB and link before moving onto the PCI bus scan, where we no longer
recover from these error conditions.
Also added is simplified documentation of the PHB reset and training flow.

than one retry.

The current implmentation has incorrect assumptions: there is
always a PCI slot associated with root port and PCIe switch
downstream port and all of them are capable to change its
power state by register PCICAP_EXP_SLOTCTL. Firstly, there
might not a PCI slot associated with the root port or PCIe
switch downstream port. Secondly, the power isn't controlled
by standard config register (PCICAP_EXP_SLOTCTL). There are
I2C slave devices used to control the power states on Tuleta.
In order to use the PCI slot's methods to manage the power
states, this does:
* Introduce PCI_SLOT_FLAG_ENFORCE, indicates the request operation
is enforced to be applied.
* pci_enable_bridge() is split into 3 functions: pci_bridge_power_on()
to power it on; pci_enable_bridge() as a place holder and
pci_bridge_wait_link() to wait the downstream link to come up.
* In pci_bridge_power_on(), the PCI slot's specific power management
methods are used if there is a PCI slot associated with the PCIe
switch downstream port or root port.

This renames the "poll" op to "run_sm" (short for run state machine).
I think this is a better name since the function does a bunch of
things like reseting the slot. Also it avoids confusion with the
"poll_link" op which does something different (and can even be called
from run_sm).
No functional change.

We has to support surprise hotplug on PCI slots that don't support
it on hardware. So we're fully utilizing the PCIe link state change
event to detect the events (hot-remove and hot-add). The PDC (Presence
Detection Change) event isn't reliable for the purpose. For example,
PEX8718 on superMicro's machines.
This adds another PCI slot property "ibm,slot-broken-pdc" in the
device-tree, to indicate the PDC isn't reliable on those (software
claimed) surprise pluggable slots.

This issue is reported from superMicro's "p8dnu" platform. PEX9733
is connected to PHB direct slot. We create dynamic PCI slots for
its (5) downstream ports and all of them support surprise hotplug
capability. The problem is power supply lost on hot-remove and it
isn't turned on automatically on hot-add. It means the PCIe link
behind the slot isn't up and the PCI adapter behind the slot can't
be probed successfully.
This fixes the issue by forcing to turn on the power supply on
hardware when user (kernel) requests to do so. Those PCI slots
are identified by additional flag (PCI_SLOT_FLAG_FORCE_POWERON).

According to PCIe spec, the presence bit is hardcoded to 1 if PCIe
switch downstream port doesn't support slot capability. The register
used for the check in pcie_slot_get_presence_state() is wrong. It
should be PCIe capability register instead of PCIe slot capability
register. Otherwise, we always have present bit on the PCI topology.
The issue is found on Supermicro's p8dtu2u machine:

This fixes the issue by checking the correct register (PCIe capability).
Also, the register's value is cached in advance as we did for slot and
link capability.

Various backends define their own PCI slot states for flexibility
with numbers [A]. PCI core also defines its PCI slot states [B].
For one specific PCI slot state, the major number of [A] and [B]
should be same so that the corresponding operation can be found.
It means [A] and [B] are relevant to some extent, but the code
where defines the PCI slots in backends doesn't reflect it.
This makes the major PCI slot state defined in backend same to
the corresponding one defined in PCI core. The minor PCI slot
states are made to be incremental to their base number (major
PCI slot state). No functional changes introduced.

This claims PCIe surprise hotplug capability through device node's
property "ibm,slot-surprise-pluggable". The slot has the capability
when surprise hotplug is supported in its slot's capability bits or
link state change reporting is supported in PCIe link capability bits.
In order for link state events to be properly raised during surprise
hotplug, the power supply to the slot should be always on. The slot's
power state should be switched accordingly during fundamental reset.

Every PCIE bridge port or PHB is expected to be bound with PCI slot
, to which various PCI slot's functionalities are attached (e.g. power,
link, reset). This supports PCI slot:
* PCI slot is reprsented by "struct pci_slot".
* "struct pci_slot_ops" represents the functions supported on the
PCI slot. It's initialized by PCI slot core at the beginning and
allowed to be overrided by platform partially or completely.
* On PCI hot plugging event, the PCI devices behind the slot are
enumarated. Device sub-tree is populated and sent to OS by OPAL
message.
* On PCI hot unplugging event, the PCI devices behind the slot are
destroyed. Device sub-tree is removed and the slot is powered off.
