Everything is now in place to use the Host IOMMU Device callbacks
to retrieve the page size mask usable with a given assigned device.
This new method brings the advantage to pass the info much earlier
to the virtual IOMMU and before the IOMMU MR gets enabled. So let's
remove the call to memory_region_iommu_set_page_size_mask in
vfio common.c and remove the single implementation of the IOMMU MR
callback in the virtio-iommu.c

They don't need to be in the global trace-events file and can have a
local trace header. Also add address_space_map tracepoint for tracking
mapping behaviour.

Emit a QMP event on receiving a PVPANIC_SHUTDOWN event. Even though a typical
SHUTDOWN event will be sent, it will be indistinguishable from a shutdown
originating from other cases (e.g. KVM exit due to KVM_SYSTEM_EVENT_SHUTDOWN)
that also issue the guest-shutdown cause.
A management layer application can detect the new GUEST_PVSHUTDOWN event to
determine if the guest is using the pvpanic interface to request shutdowns.

Shutdown requests are normally hardware dependent.
By extending pvpanic to also handle shutdown requests, guests can
submit such requests with an easily implementable and cross-platform
mechanism.

Introduce global xen_is_stubdomain variable when qemu is running inside
a stubdomain instead of dom0. This will be relevant for subsequent
patches, as few things like accessing PCI config space need to be done
differently.

Both cpu-pm and mem-lock are related to system resource overcommit, but
they are separate from each other, in terms of how they are realized,
and of course, they are applied to different system resources.
It's tempting to use separate command lines to specify their behavior.
e.g., in the following example, the cpu-pm command is quietly
overwritten, and it's not easy to notice it without careful inspection.

Since the host IOVA ranges are now passed through the
PCIIOMMUOps set_host_resv_regions and we have removed
the only implementation of iommu_set_iova_range() in
the virtio-iommu and the only call site in vfio/common,
let's retire the IOMMU MR API and its memory wrapper.

This generalises the qtest_clock_warp code to use the AccelOps
handlers for updating its own sense of time. This will make the next
patch which moves the warp code closer to pure code motion.

This is an experiment to further reduce the amount we throw into the
exec headers. It might not be as useful as I initially thought because
just under half of the users also need gdbserver_start().

sprintf() is deprecated on Darwin since macOS 13.0 / XCode 14.1.

Introduce memory_region_init_ram_guest_memfd() to allocate private

For xen, when checking for the first RAM (xen_memory), use
xen_mr_is_memory() rather than checking for a RAMBlock with
offset 0.
All Xen machines create xen_memory first so this has no
functional change for existing machines.

Always pass address with offset to xen_map_cache().
This is in preparation for support for grant mappings.

The old "-runas" option has the disadvantage that it is not visible
in the QAPI schema, so it is not available via the normal introspection
mechanisms. We've recently introduced the "-run-with" option for exactly
this purpose, which is meant to handle the options that affect the
runtime behavior. Thus let's introduce a "user=..." parameter here now
and deprecate the old "-runas" option.

Instead of using a single global bounce buffer, give each AddressSpace
its own bounce buffer. The MapClient callback mechanism moves to
AddressSpace accordingly.
This is in preparation for generalizing bounce buffer handling further
to allow multiple bounce buffers, with a total allocation limit
configured per AddressSpace.

Now we do set MIGRATION_FAILED state, but don't give a chance to
orchestrator to query migration state and get the error.
Let's provide a possibility for QMP-based orchestrators to get an error
like with outgoing migration.
For hmp_migrate_incoming(), let's enable the new behavior: HMP is not
and ABI, it's mostly intended to use by developer and it makes sense
not to stop the process.
For x-exit-preconfig, let's keep the old behavior:
- it's called from init(), so here we want to keep current behavior by
default
- it does exit on error by itself as well
So, if we want to change the behavior of x-exit-preconfig, it should be
another patch.

hw/core/cpu.h is already using struct forward declarations in some cases
to avoid inclusions, and otherwise CPUAddressSpace and CPUJumpCache
are only used together with their definition. CPUTLBEntryFull is
always used when their definition is available. Remove all three
from typedefs.h.
