This commit modifies the dwc2_hsotg_read() and dwc2_hsotg_write() functions
to handle invalid address access gracefully. Instead of using
g_assert_not_reached(), which causes the program to abort, the functions
now log an error message and return a default value for reads or do
nothing for writes.
This change prevents the program from aborting and provides clear log
messages indicating when an invalid memory address is accessed.

We pass a ResetType argument to the Resettable class enter
phase method, but we don't pass it to hold and exit, even though
the callsites have it readily available. This means that if
a device cared about the ResetType it would need to record it
in the enter phase method to use later on. Pass the type to
all three of the phase methods to avoid having to do that.

Check the value of mps to avoid potential divide-by-zero later in the function.
Since HCCHAR_MPS is guest controllable, this prevents a malicious/buggy guest
from crashing the QEMU process on the host.

Change several assert()s to qemu_log_mask(LOG_GUEST_ERROR...),
to prevent the guest from causing Qemu to assert. Also fix up
several existing qemu_log_mask()s to include the function name in
the message.

Add the dwc-hsotg (dwc2) USB host controller emulation code.
Based on hw/usb/hcd-ehci.c and hw/usb/hcd-ohci.c.
Note that to use this with the dwc-otg driver in the Raspbian
kernel, you must pass the option "dwc_otg.fiq_fsm_enable=0" on
the kernel command line.
Emulation of slave mode and of descriptor-DMA mode has not been
implemented yet. These modes are seldom used.
