block/nvme: Pair doorbell registers
For each queue doorbell registers are paired as:
- Submission Queue Tail Doorbell
- Completion Queue Head Doorbell
Reflect that in the NVMeRegs structure, and adapt
nvme_create_queue_pair() accordingly.
