PR tui/25348 points out that, when "gdb -tui" is used, then exiting
the TUI will cause a crash.
This happens because tui_setup_io stashes some readline variables --
but because this happens before readline is initialized, some of these
are NULL. Then, when exiting the TUI, the NULL values are "restored",
causing a crash in readline.
This patch fixes the problem by ensuring that readline is initialized
first. Back in commit 11061048d ("Give a name to the TUI SingleKey
keymap"), a call to rl_initialize was removed from
tui_initialize_readline; this patch resurrects the call, but moves it
to the end of the function, so as not to remove the ability to modify
the SingleKey map from .inputrc.

TUI windows no longer need to store their type -- there's only a
single spot that uses this information, and it can be changed to use
dynamic_cast. (It could be cleaned up even more, by using a virtual
method, but I haven't done so.) This patch removes the "type" field
from tui_gen_win_info, and this in turn allows removing a couple of
enumerator constants.

This removes the call to tui_show_source from tui_ui_out. This always
seemed like a hack, and now that the TUI is using the proper
observers, it seems not to be needed.
The rest of the logic remains, unfortunately, because it is needed to
suppress some gdb output in the TUI case. We could probably find a
nicer way to do this (maybe a ui_out_flag), but I haven't attempted
this.
This was the last caller of tui_show_source, so this is removed as
well.

The TUI execution info window is unusual in that it is always linked
to a source or disassembly window. Even updates of its content are
handled by the source window, so it really has no life of its own.
This patch removes this window entirely and puts its functionality
directly into the source window. This simplifies the code somewhat.
This is a user-visible change, because now the box around the source
(or disassembly) window encloses the execution info as well. I
consider this an improvement as well, though.
Note that this patch caused ncurses to start emitting the "CSI Z"
sequence, so I've added this to the test suite terminal
implementation.

tui_layout_command is a simple wrapper for tui_set_layout_by_name.
This removes the extra layer and cleans up the resulting function a
bit -- changing it to call error rather than return a result. This
necessitated a small change to tui-regs.c, to avoid calling the
function that is being removed.

While working on my other scripts to deal with gdb headers, I noticed
that some files were missing include guards. I wrote a script to add
the missing ones, but found that using the obvious names for the
guards ran into clashes -- for example, gdb/nat/linux-nat.h used
"LINUX_NAT_H", but this was also the script's choice for
gdb/linux-nat.h.
So, I changed the script to normalize all include guards in gdb. This
patch is the result.

Tested by rebuilding; I also ran it through "Fedora-x86_64-m64" on the
buildbot.

This commit applies all changes made after running the gdb/copyright.py
script.
Note that one file was flagged by the script, due to an invalid
copyright header
(gdb/unittests/basic_string_view/element_access/char/empty.cc).
As the file was copied from GCC's libstdc++-v3 testsuite, this commit
leaves this file untouched for the time being; a patch to fix the header
was sent to gcc-patches first.

This patch finalizes the C++ conversion of the ui-out subsystem, by
turning the ui_out and ui_out_impl structures into a single class
hierarchy. ui_out functions are turned into virtual methods of that new
class, so as a result there are a lot of call sites to update.
In the previous version of the patchset, there were separate ui_out and
ui_out_impl classes, but it wasn't really useful and added boilerplate.
In this version there is simply an ui_out base class that is
extended for CLI, TUI and MI.
It's a bit hard to maintain a ChangeLog for such a big patch, I did my
best but I'm sure there are some missing or outdated info in there...

The layout command supports the layout names $FREGS, $GREGS, $SREGS,
and $REGS. The intention of these layout names was to display the tui
register window with a specific set of registers.
First, these layout names no longer work, and haven't for a while, using
any of them will just result in switching to the general register view.
Second there is already the command 'tui reg GROUP' command to set the
displayed register set to GROUP, so making the layout command also
control the register set feels like unnecessary overloading of the
layout command.
This commit removes all code relating to supporting the register set
specific names from the layout command. Afterwards the user can select
an available layout using the layout command, and control the choice of
register set using the 'tui reg GROUP' command.

PR tui/16138 is about failure to initialize curses resulting in GDB
exiting instead of throwing an error. E.g.:
$ TERM=foo gdb
(gdb) layout asm
Error opening terminal: foo.

The problem is that we're calling initscr to initialize the screen.
As mentioned in
http://pubs.opengroup.org/onlinepubs/7908799/xcurses/initscr.html:
If errors occur, initscr() writes an appropriate error message to
standard error and exits.

Instead, we should use newterm:
"A program that needs an indication of error conditions, so it can
continue to run in a line-oriented mode if the terminal cannot support
a screen-oriented program, would also use this function."
After the patch:
$ TERM=foo gdb -q -nx
(gdb) layout asm
Cannot enable the TUI: error opening terminal [TERM=foo]
(gdb)
And then PR tui/17519 is about GDB not validating whether the terminal
has the necessary capabilities when enabling the TUI. If one tries to
enable the TUI with TERM=dumb (and e.g., from a shell within emacs),
GDB ends up with a clear screen, the cursor is placed at the
bottom/right corner of the screen, there's no prompt, typing shows no
echo, and there's no indication of what's going on. c-x,a gets you
out of the TUI, but it's completely non-obvious.
After the patch, we get:

While at it, I've moved all the tui_allowed_p validation to
tui_enable, and expanded the error messages. Previously we'd get:
