Fix gcc-4.5 HEAD warnings for enum ada_operator.
* Makefile.in (HFILES_NO_SRCDIR): Add ada-operator.inc.

Initialize BYTE_OFFSET to 0 by default. Explicitly check if
attr_form_is_block.

byte_offset. Fix crash on non-DW_BLOCK ATTR values.
