This patch constifies ui_out_impl in struct ui_out, and various
instances of ui_out_impl.
This removes a couple of FIXME comments (near cli_ui_out_impl and
mi_ui_out_impl) that did not make sense to me.
Tested by rebuilding.

This replaces XMALLOC with XNEW, and removes XMALLOC. The only
non-mechanical bit here was remembering to edit gdbarch.sh.

(ui_out_impl): Add a field for data destructor in ui_out_impl.

which does nothing.

default_data_destroy
(uo_data_destroy): Local function which invokes the data
destructor if present.
(clear_table): Local function which clears the table data of a
ui_out object.
