This patch provides some minimal thread-safety to BFD.
The BFD client can request thread-safety by providing a lock and
unlock function. The globals used during BFD creation (e.g.,
bfd_id_counter) are then locked, and the file descriptor cache is also
locked. A function to clean up any thread-local data is now provided
for BFD clients.

In GDB we have a problem with the BFD cache.
As GDB runs for a potentially extended period of time, if the BFD
cache holds a file descriptor for an open on-disk file, this can, on
some targets (e.g. Win32) prevent the OS writing to the file.
This might, for example, prevent a user from recompiling their
executable as GDB is (via the BFD cache) holding an open reference to
that file.
Another problem, relates to bfd_stat, for BFDs that are using the BFD
cache (i.e. they call cache_bstat to implement bfd_stat). The
cache_bstat function finds the BFD in the cache, opening the file if
needed, and then uses fstat on the open file descriptor.
What this means is that, if the on-disk file changes, but the cache
was holding an open reference to the file, the bfd_stat will return
the 'struct stat' for the old file, not the new file.
Now, for this second problem, we might be tempted to make use of an
actual stat call, instead of calling bfd_stat, however, this isn't
ideal as we have some BFDs that use a custom iovec, and implement the
various functions over GDB's remote protocol. By using bfd_stat we
can have a single call that should work for both local files, and for
remote files.
To solve both of these problems GDB has calls to bfd_cache_close_all
sprinkled around its code base. And in theory this should work fine.
However, I recently ran into a case where we had missed a
bfd_cache_close_all call, and as a result some BFDs were held open.
This caused a bfd_stat call to return an unexpected result (old file
vs new file).
What I'd like is some way within GDB that I can do:
gdb_assert ( /* Nothing is held open in the cache. */ );
As this would allow GDB to quickly identify when we've missed some
bfd_cache_close_all calls.
And so, to support this, I would like to add a new bfd_cache_size
function. This function returns an integer, which is the number of
open files in the cache. I can then start adding:
gdb_assert (bfd_cache_size() == 0);
to GDB in some strategic spots, and start fixing all of the missing
bfd_cache_close_all calls that crop up as a result.

Within bfd/cache.c change the type for max_open_files and open_files
variables from int to unsigned. As a consequence of this, the return
type for bfd_cache_max_open() is also changed from int to unsigned.
Within bfd_cache_max_open I've left the local 'max' variable as an
int, this should ensure that if the sysconf call fails, and returns
-1, then the computed max value will be less than 10, which means
max_open_files will be set to 10. If 'max' was changed to unsigned
then, should the sysconf call fail, we'd end up with max becoming a
very large positive number ... which is clearly not what we want.
And, while I was auditing how open_files is used, I added an assert
within bfd_cache_delete to ensure that we don't try to reduce
open_files below zero.
There should be no user visible change with this commit.

bfd/doc/chew.c extracts documentation from source code comments
annotated with keywords, and generates much of bfd.h and libbfd.h from
those same comments. The docs have suffered from people (me too)
adding things like CODE_FRAGMENT to the source to put code into bfd.h
without realising that CODE_FRAGMENT also puts @example around said
code into the docs. So we have random senseless things in the docs.
This patch fixes that problem (well, the senseless things from
CODE_FRAGMENT), moves most of the code out of bfd-in.h, and improves a
few chew.c features. libbfd.h now automatically gets ATTRIBUTE_HIDDEN
prototypes, and indentation in bfd.h and libbfd.h is better.

If pe_ILF_object_p succeeds, pe_ILF_build_a_bfd will have changed the
bfd from being file backed to in-memory. This can have unfortunate
results for targets checked by bfd_check_format_matches after that
point as they will be matching against the created in-memory image
rather than the file. bfd_preserve_restore also has a problem if it
flips the BFD_IN_MEMORY flag, because the flag affects iostream
meaning and should be set if using _bfd_memory_iovec. To fix these
problems, save and restore iostream and iovec along with flags, and
modify bfd_reinit to make the bfd file backed again. Restoring the
iovec and iostream allows the hack in bfd_reinit keeping BFD_IN_MEMORY
(part of BFD_FLAGS_SAVED) to be removed.
One more detail: If restoring from file backed to in-memory then the
bfd needs to be forcibly removed from the cache lru list, since after
the bfd becomes in-memory a bfd_close will delete the bfd's memory
leaving the lru list pointing into freed memory.

When reading the BFD manual, I noticed text like this:
-- Function: bool bfd_close (bfd *abfd);

'TRUE' is returned if all is ok, otherwise 'FALSE'.
The *Returns*, like the *Synopsis* in the earlier patch, is
un-info-like. It's also used inconsistently.
This patch removes all the uses of the RETURNS word and removes it
entirely from the chew scripts. Now this example reads:

In a few cases I had to slightly reword the comment. There were also

The result of running etc/update-copyright.py --this-year, fixing all
the files whose mode is changed by the script, plus a build with
--enable-maintainer-mode --enable-cgen-maint=yes, then checking
out */po/*.pot which we don't update frequently.
The copy of cgen was with commit d1dd5fcc38ead reverted as that commit
breaks building of bfp opcodes files.

This patch adds bfd_stdint.h to bfd.h, so that BFD can use size_t
where appropriate in function parameters and return values. I also
tidy a few other cases where headers are included twice.

Archive element IO is performed on the file of the containing archive,
which leads to the BFD "where" field of archives and their elements
being out of sync with the real file position. (We're talking
traditional archives here, not thin archives.) The old bfd_seek code
recognized this by not attempting to optimize away seeks for
archives. However, there was other code that could return bogus
results. For example, cache.c limits the number of open files by
closing a file and remembering its state once the limit is reached.
If bfd_tell is called on an archive element when the containing
archive is closed, it will return an invalid file pointer.
It's possible to have a valid "where" field for archives by always
using and updating the containing archive BFD. That's what this patch
does. Note that cache.c used to find the containing archive BFD
anyway for the iostream, so we're not really doing extra work, just
transferring it up to the correct abstraction level.
The patch also gets rid of some hacks. bfd_tell was called when
bfd_seek failed, in an attempt to correct "where". That's got to be
papering over another problem, so that code has been removed.
bfd_read also had an "optimiziation" to return early when the number
of bytes was zero, and bfd_seek optimized calls that didn't move the
file pointer. This was covering for a coff_slurp_line_table bug where
IO was attempted on a pe-dll BFD without an iovec.

LTO plugin support in plugin_maybe_claim wants to close the IR bfd
after replacing it with the recompiled object, but can't do so for
archive elements due to various pointers that access the archive bfd.
Thin archives have the same problem. They too cannot have their

The current hard coded limit of open files in bfd/cache.c is 10. This
is pretty low these days. Binaries are often linked against much more
than 10 files (and sometimes against more than 100 shared libraries).
When debugging with GDB some files are opened and closed multiple
times because of this low limit. If possible make the BFD cache file
limit depend on the actual open file limit of the process so more BFD
files can be open at the same time.
