* breakpoint.c (insert_breakpoints): Use ALL_BREAKPOINTS_SAFE.
(bpstat_stop_status): Likewise.
(remove_solib_event_breakpoints): Likewise.
(clear_momentary_breakpoints): Likewise.
(re_enable_breakpoints_in_shlibs): Don't reenable a breakpoint
if we still can't read the memory for that breakpoint.
(mention): Add bp_shlib_event case to keep gcc quiet.
