* symtab.c (add_param_to_type): Remove (commented-out) function,
since that functionality has been re-written.
* coffread.c: Remove commented-out add_param_to_type support.
* mdebugread.c (parse_symbol): Likewise.
* stabsread.c (define_symbol): Likewise.
