* cp-names.y (ptr_operator_1, ptr_operator_2): Merge back into
ptr_operator.
(qualified, decl1b): Remove.
(NEW, DELETE): Give higher precedence than '['.
(typespec, typespec_2, declarator, direct_declarator)
(abstract_declarator, direct_abstract_declarator): New.
(start, type): Use them.
(operator): Add rules for conversion operators.
(exp): Avoid a conflict for function-style casts; allow
function-style casts to typenames.
