* fork-child.c (SHELL_FILE): Remove #ifndef.
(fork_inferior): Remove SHELL_COMMAND_CONCAT. * gdbint.texinfo (Native Conditionals): Remove SHELL_COMMAND_CONCAT and SHELL_FILE.
