* breakpoint.c (create_breakpoint, create_breakpoints)
(break_command_really, set_breakpoint): New parameter enabled.
(create_breakpoint, break_command_really): Make breakpoint
disabled if so requested.
* breakpoint.h (set_breakpoint): New parameter enabled.
* mi/mi-cmd-break.c (mi_cmd_break_insert): Handle the -d option.
