* macroscope.h (user_macro_scope): Declare.

* c-lang.c (c_preprocess_and_parse): Always attempt macro lookup.
Use user_macro_scope.
(null_macro_lookup): Remove.
* macrotab.h (macro_callback_fn): Declare.
