Support for more than 64k ELF sections.

in multiple places. Clean up afterwards. Read symbol shndx
extension, and use with swap_symbol_in. Translate SHN_UNDEF,
SHN_ABS, SHN_COMMON and elf sections > SHN_HIRESERVE to bfd
sections too. Remove dead code.
