bfd: Rename real_{ftell, fseek, fopen}
Give these bfd-internal symbols with external linkage a _bfd_ prefix
to avoid collisions in the global symbol namespace.

* cache.c, dwarf2.c, opncls.c: Adjust all callers.
* libbfd.h: Regenerate.
