gdb: remove TYPE_INDEX_TYPE macro
Remove `TYPE_INDEX_TYPE` macro, changing all the call sites to use

gdb/ChangeLog:
* gdbtypes.h (TYPE_INDEX_TYPE): Remove. Change all call sites
to use type::index_type instead.
