* c-pragma.h: Define HANDLE_GENERIC_PRAGMAS if
REGISTER_TARGET_PRAGMAS is defined. Duplicate some
definitions from cpplib.h.
* cpplib.h: Don't typedef struct cpp_reader if c-pragma.h has
