postypes.h: Reinstate inclusion of <stdint.h>; also define the __STDC_* macros.

* include/bits/postypes.h: Reinstate inclusion of <stdint.h>;
also define the __STDC_* macros.
(streamoff): Adjust.
* include/tr1_impl/cstdint: Check that the __STDC_* macros are
not defined before defining.
