c.opt (Wmissing-declarations): Add C++ and ObjC++.

* doc/invoke.texi (Warning Options): -Wmissing-declarations now
works for C++.

* decl.c (start_preparsed_function): Add support for
-Wmissing-declarations.
testsuite:
* g++.dg/warn/Wmissing-declarations-1.C: New test.
