libstdc++: Update documentation that only refers to c++98 and c++11

libstdc++-v3/ChangeLog:
* doc/xml/manual/using.xml: Generalize to apply to more than
just -std=c++11.
* doc/html/manual/using_macros.html: Regenerate.
