Now that the Dockerfiles are included in the repo, we no longer
need a job that creates them every single time as part of the
pipeline and we can also avoid rebuilding them if their
configuration hasn't changed.

To prepare for changes in the directory structure, all the
instances of .gitignore in the project are combined together
leaving a single .gitignore file in the root directory.
