Compare pointers to nullptr, not 0. I also fixed a trailing
whitespace in the same function.
There should be no user visible changes after this commit.

Many spots incorrectly use only spaces for indentation (for example,
there are a lot of spots in ada-lang.c). I've always found it awkward
when I needed to edit one of these spots: do I keep the original wrong
indentation, or do I fix it? What if the lines around it are also
wrong, do I fix them too? I probably don't want to fix them in the same
patch, to avoid adding noise to my patch.
So I propose to fix as much as possible once and for all (hopefully).
One typical counter argument for this is that it makes code archeology
more difficult, because git-blame will show this commit as the last
change for these lines. My counter counter argument is: when
git-blaming, you often need to do "blame the file at the parent commit"
anyway, to go past some other refactor that touched the line you are
interested in, but is not the change you are looking for. So you
already need a somewhat efficient way to do this.
Using some interactive tool, rather than plain git-blame, makes this
trivial. For example, I use "tig blame <file>", where going back past
the commit that changed the currently selected line is one keystroke.
It looks like Magit in Emacs does it too (though I've never used it).
Web viewers of Github and Gitlab do it too. My point is that it won't
really make archeology more difficult.
The other typical counter argument is that it will cause conflicts with
existing patches. That's true... but it's a one time cost, and those
are not conflicts that are difficult to resolve. I have also tried "git
rebase --ignore-whitespace", it seems to work well. Although that will
re-introduce the faulty indentation, so one needs to take care of fixing
the indentation in the patch after that (which is easy).

PR tui/25348 points out that, when "gdb -tui" is used, then exiting
the TUI will cause a crash.
This happens because tui_setup_io stashes some readline variables --
but because this happens before readline is initialized, some of these
are NULL. Then, when exiting the TUI, the NULL values are "restored",
causing a crash in readline.
This patch fixes the problem by ensuring that readline is initialized
first. Back in commit 11061048d ("Give a name to the TUI SingleKey
keymap"), a call to rl_initialize was removed from
tui_initialize_readline; this patch resurrects the call, but moves it
to the end of the function, so as not to remove the ability to modify
the SingleKey map from .inputrc.

This adds "usage" text to the help for all all the TUI commands. In
some cases the usage is borderline, but I tend to think being complete
is preferable.

This changes the TUI so that the available layouts are no longer
completely hard-coded. "enum tui_layout_type" is removed, and then
all the fallout from this is fixed up.
This patch also reimplements the "layout" command to be a prefix
command. The concrete layouts are simply sub-commands now. This
provides completion and correct abbreviation behavior for free.
Finally, this also changes the name of the locator window to "status".
This matches the documentation and will be exposed to the user in a
subsequent patch.

The TUI "C-x 1" key binding removes TUI windows, based on the current
layout. With user-defined layouts, this is no longer easy to do.
This patch changes "C-x 1" to simply delete windows, leaving just the
focus window, the locator, and the command window.

The TUI "C-x 2" binding tries to switch to a different layout based on
the current layout. Once user-defined layouts are available, this
won't really make sense. I wasn't entirely sure how to handle this.
This patch changes the binding to simply cycle through the existing
layouts. I considered this a reasonable, though not ideal,
compromise.

This removes ui_file_isatty, ui_file_read, ui_file_write,
ui_file_write_async_safe, ui_file_flush, and ui_file_puts, replacing
them with calls to the appropriate method instead.

In some cases, the TUI flickers when redrawing. This can be seen
mostly easily when switching layouts.
This patch fixes the problem by exploiting the double buffering that
curses already does. In some spots, the TUI will now disable flushing
the curses buffers to the screen; and then flush them all at once when
the rendering is complete.

I'd like to enable the -Wmissing-declarations warning. However, it
warns for every _initialize function, for example:

Currently if a user starts the tui with 'layout asm' then they will be
presented with the 'src' layout.
What happens is:
1. Layout command enables TUI, selecting the SRC layout by default.
2. As part of tui_enable we call tui_display_main, which calls
tui_get_begin_asm_address, which calls
set_default_source_symtab_and_line. This changes core GDBs
current symtab and line, which triggers a call to the symtab
changed hook tui_symtab_changed, which sets the flag
from_source_symtab.
3. Back in the layout command, the layout is changed from SRC to
ASM. After this the layout command completes and we return to
core GDB which prints the prompt, however...
4. The before prompt hook is called which sees the
from_source_symtab flag is set and forces the SRC window to be
displayed. This switches us back to SRC view.
The solution I propose here is to delay installing the hooks into core
GDB until after we have finished setting up the tui and selecting the
default frame to view. In this way we effectively ignore the first
symtab changed event triggered when making main the default symtab.

This removes the call to tui_show_source from tui_ui_out. This always
seemed like a hack, and now that the TUI is using the proper
observers, it seems not to be needed.
The rest of the logic remains, unfortunately, because it is needed to
suppress some gdb output in the TUI case. We could probably find a
nicer way to do this (maybe a ui_out_flag), but I haven't attempted
this.
This was the last caller of tui_show_source, so this is removed as
well.

I noticed that "tui enable" did not correctly show the source window
as having the focus. Debugging showed that the problem was that
tui_update_variables was called after the windows were drawn, and its
result was being ignored. This changed the code to re-highlight the
windows if the value changed.

This adds readline-bindable function names to a few gdb functions that
already had key bindings. This lets users change the bindings.
This also removes the gdb-command function. Due to how this function
is implemented, it doesn't make sense to allow binding it.

This changes tui_gen_win_info::handle to be a specialization of
unique_ptr. This is perhaps mildly uglier in some spots, due to the
proliferation of "get"; but on the other hand it cleans up some manual

Readline 8.0 has a feature that lets an application name a keymap.
This in turn makes it simpler for users to bind keys in keymaps in
their .inputrc.
This patch gives a name to the TUI SingleKey keymap, so that
additional bindings can be made there. For example:

The call to rl_initialize, in tui_initialize_readline, had to be
removed so that .inputrc was not read too early. Note that Readline
explicitly documents that this call is not needed.

TUI windows keep track of their visibility in a boolean field.
However, this is not needed, because a window is visible if and only
if it has an underlying curses handle. So, we can remove this
separate field.

An earlier patch caused tui-windata.h to be essentially empty. And,
other earlier patches implemented TUI data window methods in any spot
that happened to be convenient at the time.
This patch rearranges all the data window code to be somewhat more
organized. It moves tui_data_window to tui-regs.h, and moves the
implementation of all methods to tui-regs.c. It then removes
tui-windata.h and tui-windata.c.
It also removes the "structuring" comments from tui-regs.c; these are
not the usual gdb style, and were out of date anyhow. Finally, it
moves _initialize_tui_regs to the end of the file, per the usual gdb
convention.

The TUI has a couple of special cases for updating the data window:
one in tui_rl_other_windowand one in tui_set_focus_command. As part
of the project to remove references to globals, I wanted to remove
these calls; but when I did, some simple operations (like "C-x o")
would cause the register window to blank.
This fixes the underlying problem by arranging for the data window's
refresh_window method to call the superclass method first, and then to
refresh the child windows. Then the special cases can be removed.

This changes tui_win_info to derive from tui_gen_win_info, rather than
having a tui_gen_win_info as a member. This removes a layer of member
access from the entire TUI, which is why this patch is so large. This
change will enable further removal of switches based on window type.

This introduces the tui_win_info::refresh_all method and implements it
as needed in subclasses. The name and comment are a bit of a guess on
my part. The main benefit of this patch is removing another switch on
the type of the window.

This renames the gdb exception types. The old types were only needed
due to the macros in common-exception.h that are now gone.
The intermediate layer of gdb_exception_RETURN_MASK_ALL did not seem

This commit applies all changes made after running the gdb/copyright.py
script.
Note that one file was flagged by the script, due to an invalid
copyright header
(gdb/unittests/basic_string_view/element_access/char/empty.cc).
As the file was copied from GCC's libstdc++-v3 testsuite, this commit
leaves this file untouched for the time being; a patch to fix the header
was sent to gcc-patches first.

PR tui/14126 notes that ANSI terminal escape sequences don't affect
the colors shown in the TUI. A simple way to see this is to try the
extended-prompt example from the gdb manual.
Curses does not pass escape sequences through to the terminal.
Instead, it replaces non-printable characters with a visible
representation, for example "^[" for the ESC character.
This patch fixes the problem by adding a simple ANSI terminal sequence
parser to gdb. These sequences are decoded and those that are
recognized are turned into the appropriate curses calls.
The curses approach to color handling is unusual and so there are some
oddities in the implementation.
Standard curses has no notion of the default colors of the terminal.
So, if you set the foreground color, it is not possible to reset it --
you have to pick some other color. ncurses provides an extension to
handle this, so this patch updates configure and uses it when
available.
Second, in curses, colors always come in pairs: you cannot set just
the foreground. This patch handles this by tracking actually-used
pairs of colors and keeping a table of these for reuse.
Third, there are a limited number of such pairs available. In this
patch, if you try to use too many color combinations, gdb will just
ignore some color changes.
Finally, in addition to limiting the number of color pairs, curses
also limits the number of colors. This means that, when using
extended 8- or 24-bit color sequences, it may be possible to exhaust
the curses color table.
I am very sour on the curses design now.
I do not know how to write a test for this, so I did not.

This removes a number of unnecessary casts from the TUI. Some were
found with -Wuseless-cast (which, I think, can't easily be enabled for
gdb); and some were found by inspection.
Tested by rebuilding on x86-64 Fedora 28.

In a review Pedro pointed out that interp::name is intended to be
read-only, and so an accessor would be a better fit. This patch
renames the field and adds a "name" method that is used instead.

This removes the interp_name function. It is only used a few spots --
one of which was only calling it on "this". It's simpler to remove
it; and should class interp become opaque in the future, it will be
just as easy to update the two remaining spots to use an accessor.

Currently, "layout asm" is not so useful as "layout src" with Single-Key mode:
you have to use multi-key commands like "ni" and "si" to do single-stepping.
This patch adds, in addition to "next" and "step" commands, corresponding
assembly-level ones - "nexti" and "stepi" - to Single-Key mode, with the
shortcuts of "o" (from "step Over") and "i" (from "Step Into") respectively.
